Medication Forms Are Available Online

 If your son needs to be on medication at school please print and have your physician complete the appropriate form and send it in with the medication.  All medication needs to be in its original container.  Also, please make sure it is not outdated.

Forms:

Food Allergy and Anaphylaxis Emergency Care Plan   This form is for students with an epi-pen and antihistamine for due to allergies.

Asthma Treatment Plan  This is for students on an inhaler or a nebulizer. There is a section for the physician fill out if your son has permission to self- administer his inhaler. I usually recommend keeping and additional inhaler in the nurses office in case your son forgets or loses his inhaler.

Authorization for Medication to be Given During School Hours This form is for any other type of medication that your son may need to take during the school day.
