An Important Message for Seniors and Parents of Seniors from College Guidance

Dear Seniors and Parents,
Early next week you will be receiving the Final Transcript Release Form that must be completed and signed by both the student and his parent/guardian.
Please note that this completed form is required for:
- Release of your final transcript to the college you are attending in the Fall 2013. No transcript will be sent without completion of this form by both the student and parent.
- An accurate list of the colleges to which you have been accepted for inclusion in the Graduation Program
- An accurate assessment of the total value of scholarships and grants (not loans or work study) awarded to the Class of 2013 (this information will not be student specific). Please list annual scholarships/grants by college.
Your accuracy is completing this form is critical. Please return this completed form to the Guidance Department by Thursday, May 2.
