Senior Portrait Information Form

Each rising senior is required to provide some basic information to the yearbook committee by October 26. This information will appear beside the student’s photo in the yearbook. The form, which must be filled out electronically and submitted via email to Mrs. Wink (Mrs.Wink@Hermits.com) by October 26, can be found on NetCommunity under the Document Library in the Student Information folder.
